Insurance Information

For our patients with insurance: We are happy to assist you in filing the necessary forms to help you receive the full benefits of your coverage. As we are not a party to the agreement between you and your insurance carrier, we are not responsible for how much your insurance company pays or when your claim is paid. You are responsible at the time of service for payment of any deductible, co-payment and any estimated balance not payable by your insurance company. You need to contact your insurance company to find out what your responsibility will be. If you are unsure, we recommend a consult appointment prior to surgery and we can then send in a written pre-treatment estimate to your insurance carrier and find that information for you.

The remainder of your bill is to be paid within 45 days from the date of service.
